Worksheet II Instructions

Please read through the instructions below before proceeding to Worksheet II.

1. Open and complete the Worksheet II | Activity Set-Up Form by selecting the button below. The form will open in a new window.

   If you need to leave the worksheet before submitting it, select Save draft. You can return to your saved draft by selecting My account in the main menu, then choosing Activity applications.

2. Complete and submit Worksheet II. After submitting the worksheet, close the new window and return to this page.
3. On this page, click Yes to attest that you completed Worksheet II. Then click Submit, followed by Next in the bottom right corner.
