Natalie Phillips, LCSW is the Social Worker on the Care Transitions Team at St. Luke’s Heart Failure Clinic. In this role, she supports patients navigating new cardiac and pulmonary diagnoses, connects individuals with vital community resources, and provides short-term counseling while helping establish long-term support systems.

Natalie began her social work career in Idaho at St. Luke’s Boise hospital, primarily serving telemetry units. She continues to work in the hospital setting occasionally, valuing the opportunity to maintain her acute care skills. Her professional background also includes serving as a Designated Examiner with the Department of Health and Welfare, as well as working with survivors of domestic violence and children in foster care. She holds a Master of Social Work and a Bachelor's degree in Sociology with a minor in Psychology.
