Activity Application & Required Materials

Use this page to complete the activity application, access templates and examples, and upload required materials for your educational activity.

Step 1

Complete Worksheets Part I & Part II

Begin by completing Worksheets Part I and Part II. These worksheets should be submitted first so SHAMP Office of Continuing Education staff can review your activity information and begin the accreditation process.

Select the Register or Take course button at the top of this page. When prompted, enter the access code below.

Access code: SWIDAHEC

Use Save Draft if you need to pause before submitting. If you have already submitted a worksheet and need to make changes, email [email protected].

Step 2

Use the Resource Library

The Resource Library includes templates, examples, guides, and reference documents needed to complete the application and prepare your activity materials.

Step 3

Upload Required Files

Upload completed documents and supporting materials to the project upload folder. Templates are located in the Resource Library.
