Worksheets A–C

Begin with the worksheet workflow before submitting other materials. Select Take course to the right and the access code below to get started.

  • Use Save Draft if you need to pause.
  • Use the sidebar navigation to move between sections.
  • Email [email protected] if changes are needed.
Important: Worksheets should be submitted first.

Next step after worksheets

Submit the faculty list and begin collecting headshots and bios as soon as possible after submitting the worksheets. Collecting documents from faculty is usually the longest step. Office of CE staff will use the faculty list to solicit relevant financial relationship information.

Logos

What to do: Upload logo files before publication.

Recommended sizes: course page image max width 768px; catalog image 650 × 330.

Activity or conference agenda

What to do: Upload the current version before publication.

Email communication templates

What to do: Download, edit if needed, and upload your version before publication.

30 days before activity date

Evaluation template

What to do: Review required questions and upload any additional questions at least 30 days before the activity.

30 days after activity date

Monetary tracking sheet
