Activity Application & Required Materials

Use this course to complete the application to accredit your continuing education activity and to submit required documents. Your submissions allow Office of CE staff to build your activity in the LMS and ensure compliance with accreditation requirements.

Quick start:
  1. Start with the worksheets (Register → enter access code → complete Worksheets A–C). Recommended first step
  2. Upload supporting files using the file link below (items can be re-uploaded anytime).
  3. Download the checklist at the bottom under Additional Information for a full run-down.

1) Files to Upload Timeline varies

Download templates as needed, complete them, then upload to the same file link below. You may re-upload updated versions at any time.

2) Worksheets Submit early

Select Register to begin the worksheet workflow (Worksheets A–C). If you have already registered, select Take course to resume.

How to navigate & save your work

  • Use the bottom-right navigation prompts to move through the workflow.
  • You can also use the left-hand sidebar to jump between sections.
  • If you need to pause, select Save draft before exiting.
  • If you submit and later need changes, contact CE staff at [email protected].
Important: The worksheets should be the first items submitted (faculty list may be submitted separately if needed).
